All heart conditions, no matter how complex, are managed under the Heart @ Home program at Blessed Trinity Home Health. Our skilled nurses provide detailed assessments and educational tools to help patients successfully manage medication, symptoms, mobility and other aspects of care. Patients on the Blessed Trinity Heart @ Home program are closely monitored with the use of a home tele-monitor program which gives the nursing staff vital information to help direct and manage the care delivered in the home setting. Information from this tele-monitor is also accessible by the physician who may use this critical information to adjust medications and other treatment plans. Many times, patients on the Blessed Trinity Heart @ Home program will also receive other services such as physical and/or occupational therapy, special adaptive equipment and nutritional counseling to optimize outcomes. The goal of the Blessed Trinity Heart @ Home program is to improve comfort and quality of life for those living with a cardiac condition, empower patients with the necessary skills to successfully manage exacerbations when they occur and to minimize the risk of hospitalization for these patients.
Living Well; Diabetes Management

Nurses lead the Blessed Trinity diabetes management program utilizing specialized teaching tools to educate patients and caregivers on how to successfully live with and manage diabetes.
Patients with Type 1 and Type II diabetes have different needs for education and medical supervision. Blessed Trinity’s’ skilled nurses are prepared to provide customized support for each of these conditions based on the individual patient’s needs. The education we provides is valuable whether the diabetes is newly diagnosed or longstanding, as information for the successful management of diabetes has expanded greatly in recent years and treatment is better than ever before. The Blessed Trinity nursing team focuses on each patient’s specific needs and desires as they help patients develop a life long commitment to managing their diabetes. The goal of the Blessed Trinity Diabetes program is to improve quality of life for patients living with diabetes through education. empowering patients to manage their diabetes while still enjoying all life has to offer.

For patients living with Chronic Obstructive Pulmonary Disease…
Living with Chronic Obstructive Pulmonary Disease (COPD) is stressful for patients and their families. COPD can many times cause patients and families to feel anxious and somewhat hopeless as the disease, if not managed effectively, can drastically reduce functional ability and overall quality of life. The skilled clinicians at Blessed Trinity Home Health provide care, therapies and education that are proven successful in managing COPD symptoms. Patients and caregivers are encouraged to actively participate in creating the individualized management plan and trained in the skills needed to be successful with managing COPD exacerbations when they occur. The goals of the Blessed Trinity Breathe with Ease program is to equip patients and family members with the tools and knowledge necessary to comfortably live with COPD without living in fear of an exacerbation.
Falls are the leading cause of nonfatal, unintentional injuries treated in a hospital emergency department–and the leading reason for admission to a skilled nursing facility. In the U.S. alone, one of three adults over the age of 65 falls each year, and more than half fall more than once. This annual rate increases at the age of 80 to one out of every two adults experience a fall in the home. Blessed Trinity’s Safe Steps program is a comprehensive geriatric program in which a Registered Physical Therapist evaluates the patients risk for a fall in the home. This assessment is then used to develop a systematic approach to increase the safe mobility in the home setting. The goal of the Blessed Trinity Safe Steps program is to significantly reduce fall risk for aging patients living in the home setting. This program focuses on restoration and management of gait and balance disorders to facilitate safe, independent living within the home.
The therapy staff at Blessed Trinity Home Health lead the joint replacement/revision program and focus on assessing and developing an individualized rehab plan for patients who have underwent a joint replacement or revision surgery. With the patient, the therapy staff work to develop a plan of care which promotes incision healing, pain management, and increased range of motion of the effected joint. The goal of the joint replacement/revision program is to successfully improve safety and mobility as patients return to their independence.
Chronic Disease Management/Re-hospitalization reduction program
The chronic disease and re-hospitalization reduction program at Blessed Trinity Home Health is led by our highly skilled team of nurses and focuses on helping patients successfully manage any chronic condition in the home setting. The primary focus of this program is to provide patients and caregivers with education on managing their chronic condition and through strategic activities, work to instill confidence in the patients and caregivers ability to successfully manage their health. The ultimate goal of the Blessed Trinity chronic disease management/re-hospitalization reduction program is to have an empowered patient and/or caregiver who is confident and competent to manage the ongoing aspects of their chronic disease AND to successfully manage exacerbations in a timely manner thereby minimizing the need for hospital admissions.
