- HTN (high blood pressure)
- Heart and/or Lung conditions
Medical/Surgical Team – Specializes in the care of patients living with the following diseases or treatments:
- Diabetes
- Kidney Disease
- Cancer
- General Surgery
- Neurological Conditions
- Multiple Sclerosis
- Parkinson's
- Central Venous Catheter Management
- Intramuscular Injections
- Subcutaneous Injections
- IV Infusion
- Chronic or New Wounds
- Statis Ulcers
- Pressure Ulcers
- Diabetic Ulcers
- Tracheostomy
- Colostomy
- PEG Tubes
- Collaboration with local wound care clinics
- Negative Pressure Therapy
Rehabilitation Team – Specializes in the care
of patients living with or recovering from any of the following
conditions with Physical, Occupational and/or Speech Therapy:- Joint Replacement or Revision
- Traumatic Injury
- Neurological Conditions
- Multiple Sclerosis
- Parkinson’s
- CVA/Stroke/TIA
- Head Injury
- ALS
- Frequent Falls
- Muscle Weakness
- Decreased Independence with ADL’s (Activities of Daily Living)
- Bathing
- Toileting
- Dressing
- Eating/Meal Prep
- Swallowing Problems
- Speech or Language Difficulties
